Patients are spending less and less time in the hospital, with lengths of stay being drastically reduced. Recovery, rehabilitation, and care for chronic diseases and conditions are conducted in alternate care settings. While visiting nursing makes the critical difference, agencies are forced to endure impossible government restrictions and a nationwide nursing shortage.

In response, Access 2 Care is offering an unparalleled level of service, providing innovative, comprehensive home health care services to patients across Northern New Jersey. For the first time, as part of our protocol, physicians, discharge planners and visiting nurses will be able to make recommendations regarding the frequency of therapist visits directly into our plan of care.

Patients accepted into our program, may be seen monthly, and with justification, on a weekly basis. The reason for this level of care may be to provide additional training, reinforcement, monitoring, surveillance or support.

Access 2 Care’s goal is to assist patients in returning to an optimal state of well-being and actively navigate all patients towards maximum independence based on their individual potential.
